Functional Movement Screens (with option for yearly follow-up)
Overview: Functional movement screens are an effective way to test your employee’s overall fitness level and risk of injury. The Functional Movement Screen (FMS) will assess the employee’s entire body symmetry, and balance. The screening performed by Heafner Health has been shown to be a reliable indicator for one’s risk of injury.
Optimal Candidates:
Individuals who require frequent changes in position throughout the day
Individuals who perform repetitive tasks
Individuals who perform heavy labor tasks

$250 per employee (1-3 employees)
$225 per employee (3-5 employees)
$200 per employee (5+ employees)
$200 per employee (any number with signed contract for yearly follow-up)

- Perform the Functional Movement Screen (FMS)
- Provide corrective exercises based on asymmetries and imbalances to minimize injury
- Provide employee with education regarding specific movements in their work environment
- Provide a detailed report to the employee and manager of the FMS screening and any adjustments/corrections implemented
Option of Annual Reassessment:
- Repeat 40-minute Functional Movement Screen
- Year over Year comparison of FMS score, evaluation of improvement or deteriorations
- Additional education and corrective exercises implemented based on reassessment
- Provide a detailed report of all movement pattern and risk of injury to the employee, manager, and human resources department
